There are two options for using the DCCRC Sponsorship Form:

• Click on the Print icon at the top right corner of the form below and choose a printer on your computer, tablet or phone. You can then enter the information by hand.
• Click on the Download icon at the top right corner of the form below. After the form has downloaded open it using a PDF reader like Adobe Acrobat or Nitro Reader. You can then complete the form on your computer, tablet or phone. Once complete you can print the form.

In either case please mail the form to the address shown on the form along with your cheque or money order.
